Health History and Consent Forms The person receiving treatment must complete their own health-history and consent forms so their appointment and confidential information can be recorded accurately. If they need assistance or have an authorized representative, please contact me before booking. Giving a Surprise Gift? An e-gift card is a simple, thoughtful way to give the experience while allowing the recipient to arrange their own appointment. Purchase an e-gift card in advance, and have it emailed automatically to the recipient on a date you choose. The recipient can then schedule their appointment, provide their direct contact information, and complete their personal forms. E-gift-card pricing includes an allowance for additional time and products during treatment.

Some targeted stretches are seamlessly incorporated, finished with the best head and face massage you've ever had. Your massage table will be equipped with one of the most advanced pregnancy bolstering systems available. This means that you will be able to enjoy part of your massage while lying face down.
